Advanced Notice!

St. Damian's Surgery will be closed on Monday 18th March 2019 due to staff training.

The Practice will be running an emergency only service on this day and we ask patients to only call the Surgery if they have an urgent change to an existing problem or a new problem that has presented that requires medical attention that same day.

Please bear with us should there be a longer wait on the telephone lines or at our other branch site receptions.

Update on St. Damian's Surgery

Further to the statement issued in February 2019 advising patients of the declined decision by the Wiltshire Commissioning Group to close the St. Damian's branch surgery and amend the Practice boundary we have reviewed the service that we are able to provide.

From 1st April 2019 St. Damian's branch surgery will be open on Monday, Wednesday & Friday mornings, and all day on Tuesdays and Thursdays.

Patients requiring urgent on the day appointments on Monday, Wednesday or Friday afternoons will be given an appointment at our main surgery in Bradford on Avon.

We will continue to review service provision and there may be further changes.

Please telephone the appointments team based at the Health Centre, Bradford on Avon between 8.30am-1.00pm and 2.00pm-5.30pm Monday to Friday.

Appointments can be arranged by this team to be held at St. Damian’s Surgery ( please specify site location)

To cancel your appointment

If you are unable to attend your appointment, please inform the practice by:

Telephone or TEXT message to 01225 860025 (24 hours a day, 7 days a week) - When using this service please remember to include your name and the date and time of your appointment. 

By using the 'cancel appointment' button on the home screen.

Contacting the receptionist on 01225 865200.

To use the on-line service to cancel your appointment, you can either take proof of ID to the receptionist or answer some security questions over the telephone to register. The receptionist will then provide you with your username and password to log in.  For more information about on-line appointments, please click the 'on-line appointments' tab above.

Ordering your prescription

As from 1 March 2019 the St. Damian’s Surgery reception will no longer be processing repeat prescription requests and this will now operate at the Health Centre, BoA.

You will be able to order your prescription in one of the following ways:

  • Via your Pharmacy – by dropping your request slip in at your Pharmacy with their agreement.
  • On-line – via the prescription ordering page. To register for an account you can either take proof of ID to the St. Damian’s receptionist or answer some security questions over the telephone. The receptionist will then provide you with your username and password to log in.
  • By Post – to the Health Centre, Bradford on Avon, Wiltshire, BA15 1DQ.
  • In Person – by dropping your request slip in at the Health Centre, BoA.

Collecting your prescription

If you normally collect your prescription from St. Damian’s Surgery you will now be required to collect from the Health Centre, BoA. Alternatively, we can send your prescription electronically to a Pharmacy of your choosing. To register for this service please contact your local Pharmacist or telephone the Prescription Team on 01225 866611. The Prescription Query line is open:

Monday to Friday – 9am-11am & 2pm-4pm (except the line will be closed on Friday afternoons)

Please note:

In line with CQC guidance of the safe management of medicines all prescription requests are not accepted over the telephone. Any housebound patient that has difficulty ordering their repeat prescription can contact our Transforming Care for Older People team for support on 01225 860812.
