Due to serious flooding at the BOA Health Centre on Thursday 10 January some clinics week commencing 14 January 2019 are needing to be moved to one of our alternative Practice sites.

Proposed Closure of St. Damian's Surgery

Further to recent patient consultation meetings, on the 10th December 2018, Wiltshire Clinical Commissioning Group issued Bradford on Avon & Melksham Health Partnership with further advice.

If the proposed closure of St. Damian's Surgery and Practice boundary changes go ahead in March 2019, then all patients currently registered with St. Damian's Surgery can remain with the Practice. These patients will continue to access the full range of clinical services, even if considered 'out of area' and will be entitled to home visits as they currently do now.

Please note for face to face appointments you will be expected to travel to one of our Practice sites - Bradford on Avon Health Centre, St. Margaret's Surgery or Winsley Health Centre.

To cancel your appointment

If you are unable to attend your appointment, please inform the practice by:

  • Telephone or TEXT message to 01225 860025 (24 hours a day, 7 days a week) - When using this service please remember to include your name and the date and time of your appointment. 
  • By using the 'cancel appointment' button on the home screen
  • Contacting the receptionist on 01225 898490
  • To use the on-line appointment service to cancel your appointment, please take proof of ID to the receptionist who will register you and provide you with a username and password.  For more information about on-line appointments, please click the 'on-line appointments' tab above

Quicker Check-in For Your Appointment

After the success of the self check-in machine at The Health Centre (BOA), we are pleased to inform you that now have installed a self check-in machine at St Damian's Surgery.

The new facility will enable patients to check-in quicker - especially if there is a queue at the reception desk.
